Sexual harassment is a form of discrimination based on sex/gender (including pregnancy, childbirth, or related medical conditions), gender identity, gender expression, or sexual orientation. Individuals of any gender can be the target of sexual harassment. Unlawful sexual harassment does

not have to be motivated by sexual desire. Sexual harassment may involve harassment of a person of the same gender as the harasser, regardless of

either person’s sexual orientation or gender identity.

FARM LABOR CONTRACTOR EMPLOYEE RECORD OF TRAINING
IN SEXUAL HARASSMENT IDENTIFICATION AND PREVENTION

I was provided training in the identification, prevention, and reporting of sexual harassment in the workplace.

Training was provided by:  ESPARZA ENTERPRISES, INC.

SANITATION DEPARTMENT
LOCKOUT/TAGOUT PROCEDURES

1. LOCK, HASP AND TAG STANDARD

1. All locks used for lockout/tagout will be size 3, long shackle Master Lock Co.
   lock. This standard has been set so locks used for this purpose are unique and
   easily identified.
2. Locks will be keys alike.
3. Hasps will be Master Padlock Co. part #420 (or equivalent) multi-lock hasp.
   Tags will be Brady Co. part#65502 (or equivalent) plastic coated to withstand our
   wet environment.

2. LOCK ISSUE
   
   

1. All sanitation employees involved in the repair, cleaning, lubricating, or other
   servicing of machinery will be issued one standard padlock, one hasp, and one
   tag.
2. Each employee issued a set will receive both keys to the padlock in that set.
3. These devices are for the sole purpose of lockout/tagout
4. Tags MUST be identified with your name and department

3. LOCKOUT PROCEDURE

1. Notify affected employees (Machine operator and Supervisor of area affected).
2. The employee locking out a particular piece of equipment must have the
   knowledge of the type and the magnitude of the energy, the hazards involved
   and the methods, or means to control the energy. (Electrical, hydraulic
   pneumatic, etc.)
3. Each employee in a group involved in working on a piece of equipment must
   use their issued lock/tag set (Five Sanitation employees working on equipment=five lock/tag sets.)
4. When work is completed notify affected employees.
5. Each person is to remove their own lock/tag set.
6. Test operation.
7. Release machine for use if test operation is successful.
   
   
   

4. SHIFT CHANGE

1. If equipment is being repaired or serviced, during multiple shifts or personnel are
   being relieved during shift.
2. Do not remove lockout until relief, worker installs their locks.
3. Do not "pass" keys to your lock.
4. A shift change briefing must take place.
   
   
   

5. EMPLOYEE RESPONSIBILITY

1. Your lockout/tagout set and all keys to that set are issued to you. No lockout
   device can be loaned to any other person or used for any other purpose than
   lockout/tagout.
2. The company issues your lockout/tagout sets to you at no charge. However,
   periodic lock inspections will be performed at random and any lost or damaged
   sets (outside of normal wear and tear) will be replaced, and you will be charged a
   fair market price for the replacement.
3. Lockout devices can only be removed by the employee who installed it.

6. LOCKOUT/TAGOUT REMOVAL

It will be management’s responsibility to remove any lockout device that employees have failed to remove.

1. A reasonable effort must he made to contact employee who owns the lockout
   device and determine reason device was still attached.
2. If employee cannot be contacted, a thorough safety check of an affected area
   must be performed by management staff familiar with the operation of that
   piece of equipment to determine if device can be removed.
3. If management determines that the affected equipment can be safely started, the
   device can then be removed under inspecting manager's direct supervisor.

I understand that any violation to the above policy will result in disciplinary action up to
and including termination of employment.

FOOD SAFETY POLICY

To ensure the safety of products, the following food safety guidelines must be
followed by all personnel working and accessing the fields, toilets, farm equipment,
and/or trailers, or in the warehouse. Food Safety begins on the farm and the following is
a list of guidelines that must be followed.

1. Keep working area clean and in order.
2. Do not bring any jewelry or loose objects that can fall into the product.
   Religious items may be worn if covered completely and secured to
   prevent product contamination. Management must approve any items
   warn for religious reasons.
3. All personal items must be kept away from the areas where product is handled.
4. Sickness, incidents of bleeding and other ailments (diarrhea & other communicable
   diseases) are reported to the foreman and appropriate measures are used to prevent
   product from being contaminated.
5. Rubber gloves, Metallic leak-proof band aids, or other corrective measures are
   provided and used for minor cuts or open sores.
6. Eating and chewing gum is prohibited in the fields and in the warehouse.
7. Smoking is prohibited in the fields or in the warehouse. If you smoke out of the fields or
   out of the warehouse, wash your hands before and after smoking.
8. Chewing tobacco is prohibited in the fields or in the warehouse.
9. Spitting is prohibited in the fields or in the warehouse.
10. Urinating and defecating in the field or in the warehouse is not allowed.
11. Hand washing must be done frequently and whenever hands become soiled.
    Hand washing must be done before beginning working, before and after using
    toilet facilities, after sneezing, and after smoking. Unprotected sneezing,
    coughing, spitting and nose blowing are not permitted in or around die product.
12. Trash receptacles are provided and used.
13. Harvesting containers must be used to carry only harvested product.
14. Diesel, oil, and grease must be kept from contacting raw product.
15. All product exposed to unsanitary conditions will be disposed of properly
    and immediately.
    
    
16. Glass is prohibited in the fields or in the warehouse.
17. All Employees and Visitors are required to wear hairnets, headbands, caps,
    beard covers, or other effective hair restraints. Beard nets must be worn if
    sideburns are below toe earlobes. Mustaches may be worn, however, if the
    mustache extends below the corners of the mouth, a beard net must be worn.
    This will also include handlebar style mustaches. Any visible beard growth
    is not permitted unless contained by a beard net.
18. No open-toed or open heeled shoes are permitted. Proper footwear is
    always required.
19. No items shall be carried in the shirt or blouse pockets.
20. Shirts and blouses must have sleeves that cover the underarms and completely
    cover the midriff.
21. No items shall be dangling from pant pockets or belts. For example - rags,
    handkerchiefs, key chains, watches, cell phones, pagers, etc. Company
    issued cell phones, pagers, and radios must be secured.
22. No items should be attached to work clothes, including buttons or pins.
23. Clean outer garments must be worn to protect against contamination of the product
24. Food preparation and eating during breaks and lunch breaks is not allowed in the field or
    next to the harvesting equipment.
25. The use of false eyelashes, false fingernails, or fingernail polish is not permitted.
26. No Employees with boils, sores, infected wounds or any other infectious or
    communicable disease is allowed to work in product or product material areas. All
    illnesses and lesions must be reported to a supervisor or foreman.
27. Anybody not authorized by Company Management, including vendors, are NOT
    allowed access to the facility.
28. Any employee that sees or knows of an authorized person and or an unauthorized vendor
    on the facility premises, must immediately report this unauthorized person and or
    unauthorized vendor to their lead.

CARBON MONOXIDE EMISSIONS/POISONING AWARENESS

Closed environments, such as Cold Storage Rooms that do not have adequate ventilation cause carbon monoxide emissions to build up to dangerous levels.

Exposure to unsafe levels of carbon monoxide emissions in closed environments may contribute to and/or cause death.

Other machinery that may release carbon monoxide emissions are tractors, air compressors, ATV’s and other machinery that is operated with an internal combustion engine.

Symptoms of Carbon Monoxide Poisoning

Breathing Carbon Monoxide may make you feel like you have the flu. Symptoms include Headache-Nausea-Dizziness-Rapid Breathing-Unconsciousness-Death.

When you are poisoned by Carbon Monoxide gas, you may feel weak and too confused to escape the area.

CARBON MONOXIDE EMISSIONS/POISONING PREVENTION

If you are given a duty to work in a closed environment without adequate ventilation, please immediately contact the Safety/Risk Department at (661) 831-0009.

If you are having any of the symptoms that may indicate that you are experiencing carbon monoxide poisoning:

IMMEDIATELY LEAVE THE AREA WHERE THE CARBON MONOXIDE EMISSIONS ARE TO GET FRESH AIR AND REPORT YOUR SYMPTOMS TO A SUPERVISOR SO YOU CAN GET IMMEDIATE MEDICAL ATTENTION.

If you are operating machinery, such as a Forklift, upon having any symptoms that may be due to Carbon Monoxide Poisoning:

IMMEDIATELY AND SAFELY TURN OFF THE MACHINERY/FORKLIFT AND REMOVE THE KEY AND IMMEDIATELY LEAVE THE AREA WHERE THE CARBON MONOXIDE EMISSIONS ARE EXPOSED TO GET FRESH AIR AND REPORT YOUR SYMPTOMS TO A SUPERVISOR SO YOU CAN GET IMMEDIATE MEDICAL ATTENTION.

CARBON MONOXIDE EMISSIONS POISONING

Carbon monoxide (CO) Is a dangerous gas produced by gasoline and propane powered forklift
trucks. When forklifts are operating or idling, they continuously produce CO gas. Poisoning can
occur when forklift exhaust is not removed from the working environment.

Watch for CO poisoning if gas or propane forklifts are used in the following situations:

• In cold rooms, controlled atmosphere rooms, truck trailers or shipping containers
• In sealed warehouses during fumigations
• In warehouses or enclosed areas that do not have enough fresh air
• When loading trucks that have their engine running.  Both the forklift and the truck add carbon monoxide to the air. Carbon monoxide builds up when forklifts are used indoors without enough fresh air.

You cannot detect carbon monoxide. It Is odorless, tasteless, and colorless. You can be around very high levels of carbon monoxide and not realize there is a problem. Risks increase during the Colder Weather as fresh air exposure is much more limited.

Exhaust Emissions and Your Health

CARBON MONOXIDE (CO) - Carbon Monoxide is often called "the silent killer"', because It gives no clear warning to its victims. It is an invisible gas with no taste or smell, and it will not cause any unusual feeling in the nose, mouth, or throat as it is breathed in. The first effects that are noticed are headache and faintness.
HYDROCARBONS(HC) - Hydrocarbons are a major component of smog. It can irritate the eyes, damage the lungs, and aggravates respiratory problems. Several exhaust hydrocarbons are also toxic, with the potential to cause cancer.
NITROGEN OXIDES(NOx)- Nitrogen Oxides, like hydrocarbons are a component to the formation of ozone. They also contribute to the formation of acid rain.
CARBON DIOXIDE(C02)- Carbon Dioxide does not directly impair human health, but it is a “greenhouse gas" that traps the earth’s heat and contributes to the potential of global warming.

Breathing carbon monoxide may make you feel like you have the flu. Symptoms Include:
Headache - Nausea - Dizziness - Rapid breathing - Unconsciousness – Death

When you are poisoned by carbon monoxide gas, you may feel weak and too confused to
escape the area. Find fresh air and medical attention as soon as you feel sick. Don't wait.  If you
notice co-workers who are sick, help them find fresh air.

The harmful effects of CO depend on the amount that is in the air and the length of time that
you breathe it. High amounts of CO may make you sick right away; low amounts of CO may
make you sick after breathing it for many hours.

Upon having any symptoms that may be due to Carbon Monoxide Poisoning,
immediately leave the area where the carbon Monoxide Emission are exposed and
immediately report your symptoms to a Supervisor so you can get immediate medical
attention, if you are a forklift operator, upon having any symptoms that maybe due to Carbon
Monoxide Poisoning Immediately and safely turn off the forklift and remove the key and
immediately leave the area where the carbon Monoxide Emissions are exposed and
immediately report your symptoms to a Supervisor to get Immediate medical attention.

HOW TO AVOID LIFTING INJURIES

You will be lifting, in some cases, heavy weights.

You may have a job that requires you to lift small weights but at a rapid pace.

No matter if you lift 1lb. to 60 lbs. You must lift with your PALMS UP, this
will help you avoid wrist and arm pain.

A person who lifts many times per day in a new job will get arm and wrist pain.
This is normal as your body to adjust to the increased activity for these
muscles.

This type of pain will go away without medical treatment in most cases.

If you tie bags off, then also keep our wrist up. Keeping your palm up and pulling up
prevents injuries. You will be able to work faster without pain if you do this.

I have been trained in lifting safety and fully understand that it is my responsibility to follow these safe practices. 

Any violation to the above policy will result in disciplinary action up to
and including termination of employment.

AMMONIA IN REFRIGERATION SYSTEMS TRAINING

What is Ammonia?

• Pure ammonia comes in two forms: gas and liquid.
• Ammonia gas is colorless and as a suffocating, pungent penetrating odor. It is also much
  lighter than air. If ammonia gas escapes from a refrigeration system or a storage
  container, it tends to collect in high areas or ceilings. As the amount of ammonia gas in
  the air decreases to parts per million (ppm) qualities, a natural air convention current
  spreads the gas throughout the confined area. Ammonia gas is easily liquefied under
  pressure.
• Liquid ammonia is a clear fluid that evaporates quickly at room temperature. Liquid
  ammonia also has a high compression ratio. The ratio of liquid to gas is1 liquid ammonia
  expands to form 800 liters of gas. A major ammonia spill is potentially disastrous
  because liquid ammonia evaporates quickly when exposed to air and creates an explosive
  fire hazard at high concentrations.
• Anhydrous Ammonia is a compound formed by the chemical combination of nitrogen
  and hydrogen Anhydrous means “without water”
  “Hydroscopic means it seeks water from the nearest source including the human body
  (Eyes, Nose, lungs, etc.)
• Ammonia is considered Corrosive or an Alkali

Hazards of Ammonia

• Health- Ammonia gas is very irritation go the eyes, nose, and respiratory system, which
  makes it easy to detect low concentration in the air. Ammonia is a severe eye irritant; it
  can penetrate the eye quickly, causing permanent blindness. Contact with skin or eyes
  can cause severe and potentially fatal bums. Because the gas is physically irritating, it is
  unlikely that any person will remain in an area contaminated with a detectable
  concentration of ammonia, unless the person is trapped or unconscious.
• If Ammonia contacts your eyes, skin, or mucous membranes it will quickly cause rapid
  dehydration and severe bums as it combines with the moisture of your body.
• The speed with which you wash the ammonia from your eyes or skill will determine the
  degree of injury.
• Long term exposure can cause damage to the lungs, upper respiratory tract, skin, eyes, eye lens or cornea.
• Aggravate bronchitis and asthma
• Depending on the concentration, ammonia can cause coughing, chest pain, breathing
  difficulty, bronchopneumonia, pulmonary edema, and from bronchial spasm

Ammonia does not cause cancer, does not damage chromosomal DNA, nor damages
embryonic development.

Where is Ammonia Used in the workplace?

• At home
• Refrigeration System-The refrigeration system at this facility is equipped with a
  mitigation system and an ammonia detection system.

First Aid Measures- when someone is injured in an ammonia-related accident,
first aid can help reduce the impact of their injuries and prevent further injuries from occurring.
The following steps apply to any situation in which someone is injured:

1. Do not panic
2. Ensure that there is no more danger to yourself or the victim.
3. Using appropriate safety gear, remove the victim from the contaminated area.
4. Send for medical help.

• Eyes- Eye contact with liquid ammonia for even a short period can cause
  permanent disability such as blindness, flushing must within 10 seconds
  Remove any contact lenses, Flush eyes for a minimum of 15 minutes, keeping the
  eyelid open, and cover both eyes with moist sterile gauze pads and bandage
  enough to keep light out. Medical Attention Needed
• Skin- Skin contact can result in severe-even fetal- burns. Before attempting
  to Hush the victim’s skin make sure the victim is breathing properly.
  Wash with water for a minimum of 15 mints. Do not remove clothing if frozen to
  skin. Medical attention needed
• Inhalation-Someone who has inhaled ammonia may be unconscious, and
  may have difficulty breathing or may have stopped breathing completely.
  Follow these steps when treating a victim of ammonia inhalation:
  Remove from exposure, administer artificial respiration or oxygen if breathing
  has stopped, seek medical aid Call 911

How to Detect Ammonia?
The most recognizable property of ammonia is Smell
If you smell ammonia and/ or feel eye irritation, leave the area. Notify your supervisor.
Detectors
Alarms- an effective alarm system includes a monitor (hat constantly tracks ammonia levels and
an alarm that responds if ammonia concentrations reach a certain present level.
Personal Protective Equipment: Controlling exposure requires strict attention to ammonia
exposure limits. Appropriate respiratory, eye, and skin protection are essential. Workers should
be familiar with and understand the requirements.
Eye Protection- Because eye irritation from exposure to ammonia gas normally does not occur
until concentration reach about 70pp„ eye protection is essential. Workers should be familiar
with and understand the requirements of their employer’s written exposure control plan.
Skin Protection-Workers who are controlling any liquid ammonia leak must have access to full body protective suits. This equipment must also be available to workers exposed to airborne
ammonia concentrations above 300ppm for more thana few minutes, which causes immediate
irritation of moist body areas.
Respiratory protection- Choosing the right respirator must be based on both the needs of each
individual worksite and the requirements of the employer’s written safe work procedures.

If an Ammonia leak is present (either by smell or sight):

Move away from the area quickly

Emergency procedures: Be prepared for emergencies

• Known the emergency meeting location
• Evacuation Procedure and check and ensure that all personnel are evacuated
• Notify the police, fire department, hospital, and other emergency response unite (such as
  suppliers)

First Aid Kit:

• Employees must have access to an appropriate first aid kid.

REMEMBER

If you smell Ammonia, be concerned.
Report Any Ammonia leaks immediately to your supervisor.
Know the emergency evacuation procedure and the designated Area.
